Enrollment Forms

WELCOME TO PRETTY WATER SCHOOL!

 

Only new students enrolling into Pretty Water for the first time need to complete the online enrollment form below.

All current students, PreK – 7th grade, enrolled in 2019-2020 will automatically be enrolled for the 2020-2021 school year.  A yearly update form will be sent home at the beginning of the school year.

An Open Transfer form must be completed and submitted by May 31, 2020 for students that do not reside in the Pretty Water School District. (This is the only form that is a PDF fillable and can be submitted by saving and uploading into the Enrollment Form, or submit by email as an attachment or copy link and paste into an email. )

Resident students are required to provide a Proof of Address.  This must be a utility bill, i.e. electric, water, trash and must have the address of service noted on the bill.

Other required documents for enrollment are: Birth Certificate, Immunization Record, Social Security Card and CDIB Card(if applicable).

We will send out a notification when the office is open to receive your child’s required documents.
